- To access the questionnaire, register for or log into your existing IRCC Secure Account.
- Scroll down to Start an Application and click on Apply to come to Canada. You should then be directed to the questionnaire.
- At the end of the questionnaire, you will be provided with your Document Checklist and a personal reference code (save this).

How long are you planning to stay in Canada?
“Temporarily – more than 6 months” even if you have only one semester of studies remaining.
Do you have a provincial or territorial attestation letter or meet an exception from submitting a provincial or territorial attestation letter?
- "Yes, I meet an exception from submitting a provincial or territorial attestation letter” if you will be studying at the same designated learning institution (DLI) and at the same level studies as your current study permit. - "Yes, I have a provincial or territorial attestation letter” if you will be studying at a different DLI and/or at a different level of studies as your current study permit – e.g. you are moving from a Bachelor’s level of studies to a Master’s level of studies. Your new CAQ for your new level of studies will serve as your Provincial Attestation Letter (PAL).
Do you also want to apply for a work permit?
"Yes, I wish to apply for a CoOp Work Permit" only if you are in a program that has a mandatory co-op, internship, stage, field experience component.
Do you fit into one of these categories (want to stay in Canada for more than 6 months and have lived in one or more designated countries or territories for more than six months in the past year or intend to work in one of the following jobs > health sciences worker, clinical laboratory worker, patient attendant in nursing or geriatric homes, medical student admitted to Canada to attend university, medical elective and physician on short term assignment, teacher of primary or secondary schools or other teachers of small children, domestics worker, someone who gives in-home care to children, the elderly and the disabled, day nursery worker?
“Yes” if you have visited or lived in a country or territory that requires a medical exam for more than 6 consecutive months in the past year or if you wish to work in any of the listed jobs (or related jobs – e.g. babysitting).

IMM 5709: Common Questions & Answers
UCI (universal client identification number)
The 8 to 10 digit “Unique Client Identifier” number located on your study permit. It looks like this: 0000-0000 or 00-0000-0000.

Personal Details Question 7 – Current Country or territory or residence:
- “Country or Territory” = Canada
- “Status” = Student
- “From” = Date your original study permit was issued to you
- “To” = Date your study permit expires
National Identity Document:
Answer “Yes” only if citizens in your country have a national identity document.
- Indicate a valid mailing address in Canada.
- Your new study permit will be mailed to this address.
- Document number = number on your most recent study permit.
- The number starts with a letter (F or U) followed by 9 digits.
- See the top right-hand corner of study permit under the oval.
Section: Details of Intended Studies in Canada
Question 1 – Details about your educational institution

Question 3 – Duration of expected study
- “From” = The date you are completing the form or a future date if you are applying for a study permit for a new program of study.
- “To” = Last day of your final term of studies *E.g. April 30 if the final term is Winter; August 31 if the final term is Summer; December 31 if the final term is Fall.
Question 4 – Cost of studies and funds available
- “Tuition” = Total tuition + related fees for the next academic year (refer to McGill’s Fee Calculator for undergraduate and graduate students). *If you have only one semester remaining, you can indicate your tuition and fees for your final semester of studies.
- “Room and Board” and “Other” = Indicate your estimated living expenses in the “Room and Board” section or the “Other” section. You can indicate an amount for living expenses in one of the sections (and leave the other section blank) or you can divide your living expenses between both sections.
Question 5a – Funds available for my stay and who will be paying for my studies
- The total amount of funds you are able to show (confirmed in your financial capacity documents – e.g. bank statements, departmental funding, financial aid, etc.).
- This amount should be equal or greater to the sum of your tuition and living expenses – i.e., equal or greater to the sum of “Tuition” and “Room and Board” / “Other” sections.
Question 5b – My expenses in Canada will be paid by
- Select “myself” if you will be paying for your studies through loans, bursaries, scholarships, stipends, or funding from McGill or any other institution.
- Select “other” if a parent, relative, or other person will be paying for your studies.
Question 6a and 6b – In addition to a study permit, are you also applying for a work permit? What type of work permit are you applying for?
- Answer “Yes” and select “Co-op Work Permit” only if you have a mandatory internship or work placement and want to apply for a Co-Op Work Permit.
Question 7 – If you have been issued a CAQ, provide the Certificate Number:
- Use the number on your CAQ that begins with “C00”. The CAQ should be valid beyond the date your current study permit expires.
- If your CAQ expires on the same date as your study permit and you may not have your new CAQ before your study permit expires, contact ISS right away.
Section: Education
- Enter your current studies at McGill.
- In the “To” section, enter the date you complete and validate the IMM 5709 form.
Section: Employment
- If you are currently studying at McGill, enter “Student” in the Current Activity/Occupation section, “McGill” in the Company/Employer/Facility name, and details about McGill in the remaining sections (i.e., Montreal (City/Town), Canada (Country or Territory), QC (Province/State).
Question 2 – Previous Activity/Occupation:
- Enter all employment – both inside and outside of Canada – you have held in the past 10 years.
- If you do not have any work history (or limited work history), indicate your studies and activities for the last 10 years.
- If you do not have enough space, enter details on a letter of explanation (e.g. in a Word document) and upload in the "Client Information” section of your Document Checklist.
Background Information section:
- Read each of the questions in this section carefully and answer honestly.
- If you do not have enough space, upload a letter of explanation in the “Client Information” section of your Document Checklist.
Question 2c – Have you previously applied to enter or remain in Canada?
- All applicants must answer yes to this question. Provide details in section 2d – e.g. I previously obtained a study permit which expires on X DATE.
- If you have previously applied to enter or remain in Canada more than once, mention each of these times.
Review and Validate the IMM 5709 form:
- Review the IMM 5709 form and press the “Validate” button. This should generate a bar code.
- Save your application and then upload the file to your Document Checklist.
- You do not need to print and sign this form by hand. After you upload all of your documents and pay the processing fee, you will have to submit your electronic signature.
- If you make any additional changes to your form after validating, you must click “Validate” and save again.
