You must submit this form in order to be registered for lessons.
Please refer to the list of instructors available to teach the above instrument. You may indicate up to three instructors in the drop-down menu. If you don't wish to express a preference, please select "no preference." An instructor will be assigned in all cases.
If you miss the deadline, submit the form anyway because it is required for teacher assignment and registration purposes. Your request will be considered at the discretion of the Chair of the Department of Performance of the Schulich School of Music. Late fees may apply.
