Removal Of Neuro (McGill) Tagged Equipment (reminder of existing policy)

In order to take a piece of equipment (eg, computer monitor, desktop unit, printer, camera, microscope etc. ) tagged with a McGill identification sticker, the authorized person must:

  1. Print out a letter or email with the information requested below
  2. Give this letter or email to Neuro Security to retain before you exit The Neuro

Information Required

  • Contact for Authorizer (Supervisor, Manager or Department Head)
  • Name
  • McGill Staff ID number
  • Email
  • Cellphone number
  • Contact for Person removing the equipment
  • Name
  • McGill staff or student ID number
  • Email
  • Cellphone Number
  • Details of Equipment
  • Description of equipment (eg, computer monitor, desktop unit, printer, camera, microscope etc)
  • Photo McGill equipment tag or McGill equipment tag number
  • Serial number of equipment
  • Location (room number) where equipment is normally housed within The Neuro
  • Date Removed
  • Expected Date of Return
