Sales Tax Assessment Questionnaire



Used by Billing Departments to perform sales tax assessment when:
  • Billing Department enters into an Agreement with a new Third Party
  • Billing Department is selling/providing goods/services to same Third Party but new Detail Code required

    (i.e. Either new fund or new account code (new good/service))

Complete the Sales Tax Assessment Questionnaire and save as a pdf document:

  • If you have a tax assessment, please attach the pdf version to the Billing Questionnaire with a copy of the agreement.
  • If outcome is to "contact Infotax", please infotax.acct [at] (email) the pdf version of this document and attach a copy of the agreement.
