The Department of Anatomy and Cell Biology is taking steps to assist student who need to request to defer a mid-term exam or other in course assessments, when no other supporting documentation is available.

Following the Quebec government announcement to reduce the strain on an already taxed medical system, students are no longer required to provide a doctor’s note for absences of fewer than five days for incidental illness. 

Please note supporting documents are required for absences as a result of illness where the absence is for 5 or more day and is still required for non-medical requests. Valid documentation is to be provided to the Course Coordinator within FIVE working days of the original exam date.

The decision to approve or deny a request rests with the Course Coordinator. 2) Making a false statement on this form is considered a violation of the Code of Student Conduct and Disciplinary Procedures, Section 19, Misrepresentation of Facts (a) “It shall be an offence to misrepresent material facts to another for the purpose of gaining admission to the University or gaining academic advantage or credit” and will be reported as an academic offence to a Disciplinary Officer. 3) All submitted forms will also be kept on file at the Anatomy & Cell Biology department office for future verification.
