Exam Sign-Up Deadline

For all timed assessments, students must sign up a minimum of 14 days prior to the exam date.  

Office Closure

Please note that our office will be closed on from Monday September 19 - Friday September 23, inclusively.

This closure is to allow our office to complete the data migration that is required for our new system, Clockwork. We have strategically selected this week to ensure that we will be able to launch this new system before midterms. We will continue to monitor our e-mail inboxes and voice messages for urgent requests, but most of our systems must remain offline during this migration. The Exam Center will remain open and scheduled exams will still run as planned.

IMPORTANT UPDATE: Coming soon - ClockWork

Student Accessibility and Achievement will soon be implementing ClockWork (to be launched by the end of September 2022), a new online platform that will make it easier for students to request academic accommodations and other support.

For requests submitted ON or BEFORE Sunday October 2, 2022, students can continue to use the form below. Submissions received will then be transferred to the ClockWork system.

For submitted ON or AFTER Monday October 3, 2022, students will be required to submit their request using ClockWork. Further information about this system will be provided in the coming weeks.

 

Mentee Application Form

When matching, we prioritize pairing mentees with mentors who are in the same program as them, and further in along in their academic path. While we cannot guarantee the availability of mentors based on specific preferences, they can be taken into consideration when pairing. 

*Please note that all information shared in this form may be shared with your mentor at the time of pairing. We will never disclose your diagnosis with your mentor, nor any other personal or medical information deemed to be confidential.
