If you require a form to be completed for a centralized application service (i.e.: Common Application Transfer College Report, AMCAS, CGFNS, LSAC, SOPHAS, WES, …), you will need to access the ES Services (Enrolment Services eStore & MyCredsTM) Menu in Minerva to upload your document and submit your request. Your document will be processed and deposited for sharing with a third party via MyCreds™. A $15 fee per share is applicable when using MyCreds™.
