Nomination Form: Faculty Honour List

Please make sure to attach the following documents in a compressed (zipped) folder or in a Adobe PDF Portfolio:

  • Two letters of support, including one from a divisional chief or departmental chair, and one from a colleague or former student. The letters should specify the nominee’s contributions to teaching; educational leadership and innovation; faculty development; and research and scholarly activity in medical education. Letters of support from students should be from past students only, not current students.
  • An electronic curriculum vitae, only including elements relevant to this award. For example, grants or publications that have no reference to education should not be included.
  • A teaching dossier highlighting the nominee's educational accomplishments (including a description of teaching roles and responsibilities, student evaluation of teaching - organized and compiled from the past 10 years only - and a description of educational innovations).   • A brief description of the nominee's future plans as an educator.
