Nomination Form

Reference Period:  Deadline to Submit:
January 1, 2025, to December 31, 2025 Deadline: March 20, 2026, at 5:00 PM ET

A complete nomination consists of the following:

  • Nomination form;
  • Nomination letter, explaining why nominee should be considered for the award (NOTE: Nominees may not self-nominate.);
  • Statement from nominee, consisting of a list and brief description identifying measures taken by nominee in respect of equity and/or community building;
  • Up to four additional letters of support (from students, former students, colleagues, Chairs, Directors, supervisors, etc.).
