Medical Note Submission Form for Trainees

(for sick leaves exceeding 4 days)

PGME is pleased to introduce a new Medical Note Submission Form for residents and fellows.

Effective September 24, 2026, PGME will require medical notes for sick leaves exceeding four (4) consecutive days.

Trainees are asked to submit their medical note using the new form. This process provides a standardized, secure, and confidential way for trainees to submit medical documentation directly to the PGME Office.
