Request for Late Course Add Form (After the Add/Drop Deadline)
It is every student’s responsibility to ensure that any record changes, such as course registrations, are completed by published University deadlines. A lack of awareness of deadlines is not, in and of itself, a valid reason for approving a late course addition. Requests will be reviewed by the Internships & Student Affairs Director on a case-by-case basis. If approved, an administrative fee of $50.00 will be charged to your Student Account in Minerva.
Instructor Approval
Instructor approval is required for all courses except EDFE (Field Experience) + the associated Professional Seminar, and EDEC 215 (English Exam for Teacher Certification). DO NOT EMAIL THE INSTRUCTORS FOR THESE COURSES. ISA is responsible for approving.
To obtain Instructor Approval:
Complete the form below and enter your instructor's email address correctly. The email your instructor receives will include your name, student ID, and your description of why you want to add the course. The instructor in turn will be asked to indicate:
• Have any assignments been required, to date? Yes/No
• Have any tests been written, to date? Yes/No
• Can you confirm that the student has been attending class since the beginning of term? Yes/No
• Will you permit this student to register late? Yes/No
