Your Risk Assessment Session

At your visit/counselling session:

Please remember to bring your Jewish General Hospital card and your Quebec Medicare (RAMQ) card with you on the day of your appointment.

When you arrive, you will be asked to complete an intake questionnaire. The doctor will then review your medical and family history information and provide a personalized risk assessment that includes short-term and lifetime risk of developing breast cancer.

A clinical breast exam is offered, but not mandatory. You will then be provided with a summary of our recommendations for your own records, and a copy will be sent to your family doctor. Recommendations will include age of onset and frequency (annually or every two years) of mammographic breast screening, as well as whether you should be offered additional imaging (i.e. mammogram and ultrasound or digital breast tomosynthesis).

*** Please note that the Breast Cancer Risk Assessment Clinic is not currently accepting new referrals due to overcapacity.

Thank you for your understanding. ***
