Instructions for filling out proof of insurance requests

  1. Download and complete either the standard proof of insurance request form or the student proof of insurance request form. Please keep the document in Word format.
  2. In the first section, Requester Information, please only fill out Requester Name and Department. Please do not modify the pre-populated remaining fields in this section.
  3. For the standard form: Under the second section entitled Certificate Holder Information (Third Party), indicate the information of the third party. This information pertains to the company or institution requesting the proof of insurance.
  4. For the student form: Under the second section entitled Certificate Holder Information (Third Party), please indicate the information of the third party hosting the student. This is the company or institution requesting the proof of insurance.
  5. The third section, Insurance Coverage Requested, refers to the insurance coverage of which the third party is requesting confirmation. It is usually Commercial General Liability (the first coverage listed). If you are unsure, you can leave it blank as our office will review this section and validate it. Send us a copy if you have an agreement that states the specific insurance requirements.
  6. The same goes for the fourth section, Additional Insureds / Interests. If you are unsure, you can leave it blank as our office will review this section and validate it. If needed, we will ask for documentation to confirm.
  7. In the last section, Description / Event Details / Reference / Special Instructions or Requests, please indicate the student’s name, if applicable, a brief description of what the certificate is for (for example, an event) and any dates for which the coverage is needed (for example, date of event).

Please submit your certificate of insurance request and a copy of the related contract by email to supportrisk.insurance [at] mcgill.ca at least two weeks before the certificate is required to allow sufficient time for us to assess and process the request.

Please include any supporting documentation, such as an agreement or contract, where applicable.
