Submit this form if you need to add, substitute or delete a vehicle from your department’s fleet. The form needs to be submitted by the vehicle asset steward.

This form needs to be updated and submitted to RAIS annually by each unit to ensure accurate records. If there is a change throughout the year, please use the form to send us your update.

Joint report of automobile accident form
In the event of an accident involving another vehicle, the driver must fill out the joint accident report. The form should be submitted to RAIS as soon as possible.

Miscellaneous property insurance form
This form needs to be filled out to request insurance for property that is rented, loaned, borrowed, in transit, or that requires specific insurance coverage.
Certificates of insurance
A certificate of insurance is a proof that insurance is in effect. It is produced by an insurance broker on behalf of the insurance company. It is requested when:
- The University is actively involved with another institution.
- The University either participates in or organizes an event off campus.
- A student is participating in a University-approved activity off campus and their host institution requests a proof of liability coverage.
- The University hires a third party for services such as events, e.g., large tents for Convocation.

Standard proof of insurance request
This form is for official McGill events off campus and third-party requests for proof of insurance. Requests must be completed and sent to us by a faculty or staff member.
Please include any supporting documentation.

Student proof of insurance request
This form is for students in placements, electives, stages, or rotations whose host institution requires Proof of Insurance.
Students who need to provide proof of insurance should ask the relevant manager within their academic unit to complete the online form for them.
Please include any supporting documentation such as a contract or agreement.
Notes
Please submit your certificate of insurance request and a copy of the related contract at least two weeks before the certificate is required to allow sufficient time for us to assess and process the request.
Join any supporting documentation and inform us whether a legal review has been undertaken.
We only review the terms of the insurance clause, other terms in the contract should be reviewed by Legal Services.
Contract and certificate of insurance review
Contracts and agreements are important methods of transferring risks to other parties. The insurance terms in a contract can assist in transferring risks. Risk Advisory and Insurance Services can review insurance language found in contracts, as well as a third party’s certificates of insurance. This includes all types of contracts involving research, equipment, leases and venues. Contracts should always refer to the University as The Royal Institution for the Advancement of Learning / McGill University.
If you need to have the insurance terms of your contract reviewed, supportrisk.insurance [at] mcgill.ca (please send us a copy). If applicable, please include a copy of the third party’s certificate of insurance for our review. For research contracts, a Research Checklist form will also need to be filled out (restricted to authenticated McGill users only).
