Credentialing Made Easy

All verification requests must include the following two (2) items:

1. Online request and payment

The request and payment are done online through our Credentialing E-store. Once the request and payment have been received, you will be able to track the status of your credentialing request by using the “view request status” link provided in the confirmation email you will receive.

Fees are as follows:

Verification of training dates and program ONLY on McGill letterhead


Verification of training dates, program and performance on form


Certificate duplicate or replacement


Please note that we do not have records for training that occurred prior to 1972. To verify training that occurred prior to 1972, please submit a request to the DPS office of the hospital where the training occurred.

2. Signed authorization to release information by the physician

In order to proceed with any request, the Authorization Form must be completed and signed. It can be mailed, faxed or emailed:

Processing time

Please note that requests for verification will only be processed once the appropriate fee and the signed authorization to release information by the physician have been received.

If we have not received them within 3 months after the date of your application, your request for credentialing will be cancelled without refund.

For electronic files, after 2009, It takes 4 weeks to process a complete request. For Paper files, before 2009, It takes 4-6 weeks to process a complete request.
