Submitting the Application Form(s)

To facilitate the accreditation/certification eligibility review process, ensure you are completing the most recent application form and you provide accurate answers and full transparency to all questions.

1. MOC Section 1 and Mainpro+ Group-Learning Activities

A minimum of 25% of the activity must be defined as interactive learning:

• Collective Application form for MOC Section 1 and Mainpro+ credits

2. MOC Section 3 Activities Self-Assessment (SAP) or Simulation Activities

In addition to the Collective Application form for MOC Section 1 and Mainpro+ credits, select the pertinent application form below:
