Disability Claims, Parental Leaves, Workplace Accommodation Requests

To share your medical information in a secure manner, we recommend uploading your documents using an MS Form with your McGill credentials. Your medical certificates can be submitted to any of the following:

• Your supervisor or departmental administrator, and/or

  • Department Form

• Your Faculty of Science HR representative, and/or
  • HR Faculty Form
• Disability Management (Central HR)
  • Central HR Form

After submitting the medical certificate, please confirm with your manager or HR representatives the submission of the documents and steps regarding your claim.

We strongly recommend using those methods and avoiding the use of email, as it is not a secure method of sharing private information.

Attending Physician's Statement

In order to be eligible for short-term disability leave, you must submit medical documentation attesting of a disability. The documentation must be completed and signed by a qualified physician or a registered nurse practitioner.
