Creating and adapting early psychosis measures for more diverse, global contexts

Iyer has been collaborating with the Schizophrenia Research Foundation in Chennai, India since 2006. This collaboration involved a large comparative study of multiple outcome domains and family factors in early psychosis between persons receiving early intervention services for first-episode psychosis in a low-middle income (Chennai, India) and a high-income (Montreal, Canada) context. This study was supported by the National Institutes of Health, U.S.A.

For this study, the collaborating teams created measures of novel constructs (e.g., What level of responsibility for recovery would you assign patients versus their families versus their treatment providers); and adapted existing measures to make them relevant in both contexts (e.g., references to going to church or playing rugby were replaced with more locally pertinent references) and to make them more youth-friendly (e.g., added video games in a measure of recreational activities). Creation and adaption involved various steps such as a review of the literature, qualitative methods and consultations with various stakeholders such as persons with lived experience, staff and researchers from both teams.

All measures were also translated and back-translated from English into Tamil and Canadian French using best practices. Many of these new and adapted measures were examined for their psychometric properties such as test-retest reliability – do people respond to measures similarly if they fill out this week and then two weeks later?; and validity – are scores on the new measure associated with scores on a similar but well-established measure?).
