Prepare for Your First Visit

To all patients who have had investigations conducted at an institution other than the MUHC, it is absolutely necessary that you bring a CD of all recent scans relevant to the issue under investigation to your first visit; this includes CT/PET scans, MRIs and/or ultrasounds. In addition, please bring with you the following (applicable) documents:

  • Biopsy results/Pathology report
  • CT Chest/Abdomen/Pelvis scan results
  • CT Head/MRI Brain scan results (if applicable)
  • Endoscopy/Gastroscopy report
  • PET Whole Body scan results

Suspected lung cancers

  • Biopsy results/Pathology report
  • CT Chest/Abdomen/Pelvis scan results
  • CT Head/MRI Brain scan results (if applicable)
  • PET Whole Body scan results
  • Pulmonary Function Test

  • Barium swallow results
  • Biopsy results
  • Bronchoscopy/Bronchoalveolar lavage
  • Endobronchial ultrasound results
  • Endoscopic ultrasound results

Please note: Our administrative team works hard to prepare your chart prior to your first visit, however sometimes, it is not possible to have all documents in order. Any investigations conducted at outside institutions are necessary to provide you with efficient and best possible care; therefore, should these documents be unavailable at your first visit, please expect delays.
