For patients managing multiple chronic conditions, seeing different specialists can mean multiple appointments, long waits and hours spent travelling between clinics. But what if three specialists could see a patient together, in a single visit?
That's the idea behind DECIDE-CV, a multidisciplinary program co-led by Dr. Abhinav Sharma, Associate Professor in the Division of Cardiology, alongside Dr. Michael Tsoukas, Associate Professor in the Division of Endocrinology and Metabolism, and Dr. Thomas Mavrakanas, Associate Professor in the Division of Nephrology.
The team, previously recognized among the Department's Emerging World Class Teams in 2024, was recently named a finalist for the 2026 Prix Hippocrate in the highly competitive Transformative Project category of the Jean-Paul Marsan Innovation Award.
Launched at the MUHC in 2020, DECIDE-CV is Canada's first synchronized cardiovascular-renal-metabolic care clinic. The program has since cared for more than 500 patients, cut the time spent accessing care, including travel, and trained more than 30 trainees. Its model was also cited in the 2026 AHA/ACC/ADA/ASN guidelines on cardiovascular-kidney-metabolic syndrome.
The recognition marks another milestone for DECIDE-CV and shows what is possible when specialists work across traditional boundaries to better meet patients' needs.
