The introduction of the Varian Truebeam linear accelerator and its developer's mode framework has enabled new strategies for treatment delivery. Under this framework, simultaneous motion of the gantry, couch, and collimator during radiation delivery can be programmed to generate highly customized treatment plans. For example, translational couch motion can be used to shorten the treatment distance to take advantage of a smaller projected leaf size at the target volume, or extend the treatment distance to make use of larger field sizes, potentially avoiding field junctions. Rotating the treatment couch may help to avoid irradiation of critical structures.
