Access log.

This form is to collect basic information concerning your presence on campus premises during the gradual resumption of research activities in the context of the COVID-19 pandemic. The data will be used to make sure work shift schedules are respective according to approved plans. Remember to self-report a health questionnaire before each shift. We encourage you to keep a personal proximity log of the people you have shared space with the most during your shift: this is simply for convenience and reference, for you to share with public health authorities in their contact tracing procedures, if they so require.        

This is in the interest of the health and safety of everyone.     

Please note: this form is for individuals working on campus only. Affiliated research institutes have put in place their own specific guidelines and procedures.

Thank you for filling out this form immediately after your shift. You will be able to enter information concerning the past two days only.

Non-compliance may result in the cancellation of your access privileges during the pandemic.
