Please submit a separate form for each Aid Fund Code (AFC) you require. All fields with an * must be completed.
Important Notes:
- An Aid Fund Code will be created within 5 business days of the request. The request must be completed accurately and the Finance Fund must exist in order to meet this commitment. Also note that Aid Fund Codes will not be created for Fund codes outside your jurisdiction. For example, a Department user cannot request an Aid Fund Code for a Faculty level fund.
- Requests for Named awards must adhere to the guidelines prescribed in the
Policy Related to the Naming of University Assets document.
