PIs with labs on campus premises are requested to file the following form only if they wish to report an incident related to the research activities within their respective labs.
Failure to file this report may revoke access to the PI and their staff and trainees. This report is for our PIs to demonstrate and maintain commitment towards health and safety of everyone on campus.
All fields are mandatory.
