The application process for the Leadership Development Program is two-fold.
- You must submit a completed application form.
- You must have your Departmental Chair/Director of your School/Program Director/Divisional or Site or Unit Director/Supervisor send an email endorsing your candidature and providing a FOAPAL # as a deposit.
Only those applications with all criteria completed will be submitted for final review.
Phase One of the application process:
Please fill in the application form, giving careful thought to your responses. For your convenience, there are two methods by which to complete the application form. Either download, complete and email the fillable PDF, provided here (please note that handwritten forms will not be accepted), or you can fill-in and submit the online form at this link.
Deadline:
The firm deadline for applying is Friday, December 15, 2017.
Cost:
There is no charge for this program!
Deposit:
A FOAPAL # is required as a deposit. A charge of $350 will be applied only if the applicant fails to withdraw from the program before the cancellation deadline of January 12, 2018.
Cancellation:
Only EMAIL cancellations received by the Faculty Development Office (facdev.med [at] mcgill.ca) before the deadline of January 12, 2018 will be processed without penalty. Any cancellation request received after this date will be subject to a fee of 350$.
Attendance:
It is mandatory that all applicants attend the full 3.5-day program. Please do not apply if you are unable to attend the full program.
Phase Two of the application process:
All applicants must have their Departmental Chair/Director of your School/Program Director/Divisional or Site or Unit Director/Supervisor endorse your candidature and provide a FOAPAL # as a deposit securing your place in the program. Please have your Departmental Chair/Director of your School/Program Director/Divisional or Site or Unit Director/Supervisor or their assistant/coordinator send the Faculty Development Office an email (facdev.med [at] mcgill.ca) including that you have the support of your Unit Head to attend the full 3.5 day program and providing the FOAPAL # as a deposit.
Once all of these criteria have been met, your application will move on to the review process. All candidates will be contacted by December 22, 2017.
