Desktop Service Request (hardware & software)

This form is to be used to make a request ONLY to set up a new computer, reassign or removal computers, or to install software or other devices. Please fill out one form per request.

NOTE: IF YOU ARE EXPERIENCING A COMPUTER PROBLEM OR YOUR REQUEST IS TIME SENSITIVE, PLEASE CALL THE IT SERVICE DESK AT (514) 398-3398.

Requests for set up, reassignment, or removal of computers must come from an authorized contact. Contact the IT Service Desk if you do not know who the authorized contact(s) is for your department.

If your request is for the installation of a non uPrint printers, you must first request approval from Procurement Services. Refer to Request to purchase a non-uPrint printing device.
