To all members of the Supplemental Health and/or Dental Plans
If you have incurred any health and/or dental expenses for the period from January 1 to December 31, 2004, you have until March 31, 2005 to submit your claims to Manulife Financial for reimbursement.
Please note that claims must be received at Manulife Financial's offices no later than March 31, 2005. Claims received by Manulife Financial after March 31, 2005 will not be accepted.
To ensure your claims are received in time, it is advisable that you send them to Manulife Financial well in advance of the March 31 deadline.
For those of you in Montreal who prefer to deliver your claims in person, there are two drop-off locations for your convenience:
- 999 de Maisonneuve Blvd. West, 14th floor
- Manulife Group Benefits Office at 2000 Mansfield, 5th floor
Supplemental Health and Dental claim forms may be obtained on the Benefits website, from your Area Personnel Officer/Representative, directly from the Benefits Office at 688 Sherbrooke Street West, 15th floor, or by calling the Benefits Office at 514-398-4900.
