SIS Access Removal or Reinstatement

This form should be completed by the Chair, Dean, Director, Associate Dean, Associate Director, Registrar, Associate Register, Assistant Registrar, or Manager of the unit. Otherwise, your request will not be processed and may not get a response.

You can use this form to:

  • Revoke your employee’s SIS access (TNT).
  • Reinstate access to a suspended account.
  • Set an effective date for removal or reinstatement of SIS Security access.
  • Other, Security Class, Functions, etc.
