Advanced care planning

Advance care planning is a process of reflection and communication, a time for you to reflect on your values and wishes, and to let others know your future health and personal care preferences in the event that you become incapable of consenting to, or refusing treatment or other care.  To learn more about it, check out Advance Care Planning.

As part of advanced care planning, Living Wills (or advanced health care directives) are an integral part of communication your wishes.  Within the McGill University Health Centre, one of the most used living wills is available at The Jewish General Hospital and can be obtained by accessing Living Wills.
