Self-declaration form for McGill staff

The self-declaration form must be completed by employees who are required to be on campus to provide necessary activities and functions, but are unable to do so because they must self-isolate.

The self-declaration form is required in order to safeguard the health and safety of employees and restrict the outbreak in the McGill community and the general population.
 

Employees who have access to Minerva

Please use the self-declaration tool found in the Employee menu.

  1. Log into Minerva
  2. Click on the Employee tab in the top menu
  3. Click on the link called “COVID-19 (Coronavirus) employee self-assessment”
 

Employees who do not have access to Minerva

  1. Please use the PDF icon self-declaration form
  2. Once completed, please send the form to your immediate supervisor by email

In both cases, the supervisors will send the form to their Human Resources representative.

All information will be treated confidentially. Supervisors, Human Resources representatives and the Disability Management Unit (HR) must keep the information collected safely. Any personal information must not be disclosed unless as required by law, or with the employee’s express consent.

 

Employees with a weakened immune system, chronic condition, or aged 70+

Employees required to work on campus to provide necessary activities and functions, but who have a weakened immune system, chronic condition or are aged 70 years and older, should remain at home and must send an email to disability.hr [at] mcgill.ca to inform them of their situation. The Disability Management Unit (HR) will contact them to document their condition.

 

Confirmation of illness

Any employee who has received a clinical diagnosis of COVID-19 must complete thePDF icon confirmation of illness form and send it by email to disability.hr [at] mcgill.ca. The Disability Management Unit (HR) will contact them to document their condition.

Self-declaration form for McGill students

If you have been diagnosed with Coronavirus (COVID-19) please use the self-reporting form for students found in Minerva.

  1. Log-in to Minerva
  2. Select the Student menu
  3. Select “Coronavirus (COVID-19) Self Reporting”

Information collected will be kept safely and confidentially: any personal information will not be disclosed unless as required by law or with the student’s express consent.
