Daily health check form

Employees must fill out the COVID-19 Daily health check form every day prior to entering any McGill building. This applies to all employees (including student employees). The disclosure of exposure or illness is required in order to safeguard the health and safety, and limit transmission in the McGill community.


  1. Log in to Minerva from a computer, tablet, or smartphone
  2. Click on the Personal tab in the top menu
  3. Are you required by government directives or public health officials to self-isolate (i.e. you are not allowed to come to campus)?
  4. Indicate if you are an employee or a graduate student
  5. Enter your immediate supervisor's email address
  6. If you are an employee, select the name of your Faculty or department
  7. Answer the following questions:
    1. Are you experiencing ANY of the following symptoms?
      • Fever (oral temperature 38.1°C (100.6°F) or higher)
      • Sudden loss of sense of smell (anosmia) without nasal congestion, with or without loss of taste
      • Recent cough or worsening of a chronic cough
      • Shortness of breath
      • Trouble breathing
      • Sore throat
    2. Are you experiencing at least TWO (2) of the following symptoms?
      • Stomach ache
      • Nausea or vomiting
      • Diarrhea
      • Major fatigue
      • Significant loss of appetite
      • Generalized muscle pain (not related to physical exertion)
      • Headaches
    3. Are you required by government directives or public health officials to self-isolate (i.e. you are not allowed to come to campus)? 
    4. Are you in contact with someone who has COVID-19?
      * This question does not apply if you have been in contact with a COVID-positive patient in your capacity as a healthcare worker wearing appropriate PPE.
    5. Do you live with a symptomatic individual awaiting a test result for COVID-19?
  8. Click on the "Submit" button

Should your answer be "yes" to any of the questions, you must stay home.

An email will be generated and sent immediately to your supervisor informing them of the answers provided.

All information will be treated confidentially. Supervisors, Human Resources representatives and the Disability Management Unit (HR) must keep the information collected safely. Any personal information must not be disclosed unless as required by law, or with the employee’s express consent.
