Travel Services has designated travel agencies that will provide you with travel planning services.
PAYMENT USING FOAPAL FOR AIRLINE TICKETS NOW AVAILABLE THROUGH BOTH DESIGNATED TRAVEL AGENCIES:
To process payment for airline tickets using your FOAPAL please print and complete the appropriate form -note each travel agency has its own version.
Vision 2000_authorization_form
When completed, fax directly to the travel agency. Tickets will not be issued against a FOAPAL unless the form is properly completed and signed by all parties.
