If you do not have a permit in your possession, you will need to visit our office in order to obtain one. In this case, please do not use this form. Instead, print out the Payroll_Deduction_Application.pdf and bring this to our office when you come to sign for your permit. The issuance of parking permits is subject to availability.
If you have a permit and you are returning from a university recognized leave, please complete and submit this web form. Payroll deductions will commence as of the date you stipulate on this form, which must coincide with your pay date.

Employees paid bi-weekly on Fridays
The Friday pay date covers the current week and previous week. For example, commencing deductions on Friday, December 22, 2017 would cover parking for the period of December 10 to 23, 2017. Employees paid bi-weekly on Thursdays
The Thursday pay date covers a two week period two weeks prior to the pay date. For example, commencing deductions on Thursday, December 28, 2017 would cover parking for the period of December 3 to 16, 2017. The permit would be activated as of December 3, 2017.
