Cancellation of payroll deductions form

Upon submission of this form, the permit holder agrees to cancel his/her parking privileges and payroll deductions will be ceased as of the date stipulated on submitted form, which must coincide with the permit holder's pay date. The pay schedules are available on the Human Resources web site and examples are indicated below. The parking permit presently in the staff member's possession must be returned to the Parking and Transportation Services office upon cancellation of parking privileges. If the permit holder reapplies for parking in the future, he/she is not guaranteed a permit nor the same location should a permit be available.

If you are cancelling parking due to a University approved leave, such as a sabbatical or medical leave, please submit the suspension of payroll deductions form instead.

Some examples are provided below to assist permit holders with determining an appropriate cancellation date:

  • Academics, Management, Executive and Excluded staff members: The Friday pay date covers the current week and previous week. For example, the pay date of Friday, March 7, 2014 covers parking for the period of February 23 to March 8, 2014. Entering a cancellation date of March 8, 2014 would result in the permit being deactivated as of March 9, 2014. The last parking deduction would be on the pay of March 7, 2014.
  • MUNACA, AGSEM or AMURE members: The Thursday pay date covers a two week period two weeks prior to the pay date. For example, the pay date of Thursday, March 13, 2014 covers parking for the period of February 16 to March 1, 2014. Entering a cancellation date of March 1, 2014 would result in the permit being deactivated as of March 2, 2014. The last parking deduction would be on the pay of March 13, 2014.
  • Employees who are paid weekly: The Thursday pay date covers a one week period prior to the pay date. For example, the pay date of Thursday, March 6, 2014 covers parking for the period of February 23 to March 2, 2014. Entering a cancellation date of March 2, 2014 would result in the permit being deactivated as of March 3, 2014. The last parking deduction would be on the pay of March 6, 2014.

Parking privileges cannot be cancelled retroactively. Every effort will be made to cancel the permit as per the date indicated on the submitted form however deduction cessation is subject to Payroll Services deadlines. If the deadline is missed, the deductions will be cancelled for the next pay period. If possible, please try to submit this form one month before the cancellation date to ensure that the deductions will be ceased on the date indicated on the form.
