Release of Information

All students requesting copies of documentation from their file must complete the following form. Most requests are completed within 5-7 business days.

Please note that test results can never be released over the phone nor by email (only fax, pick-up or standard mail).

Note: The time to process requests is longer if you are requesting recent test results.
