Service Point offers a Commissioner for Oaths service during regular business hours. This service is made available specifically for the following matters:
Replacement diplomas: Your sworn affidavit certifying that your original diploma was lost. The affidavit should include your full name, address, phone number, date of birth, your degree and the year it was granted. You must clearly state your reason for requesting a replacement.
Tuition-related legal documents: For students wishing to prove Quebec Residency for tuition purposes, sworn statements may be required for certain cases. Students may sign their sworn statements in the presence of one of the Commissioners for Oaths available at Service Point.
Please note this service is not available for issues other than for the ones listed above.
