Varsity Registration and Pre-participation Medical Screening Questionnaire
All returning varsity athletes, new recruited athletes and ‘walk-ons” are
required to complete the following administrative tasks before being
permitted to take to the field of play.

Varsity Athlete Registration and Biographical Information
Questionnaire. For all varsity athletes (including developmental, 'B'
Team and novice programs).
- All varsity athletes MUST complete his MANDATORY online
questionnaire, which can be found on MINERVA in the Student Menu. The
questionnaire MUST be completed BEFORE your first practice or
you will not be permitted to participate.
- The first 25 Registration questions are MANDATORY for ALL varsity
athletes. Although you can choose to complete the questionnaire in a
single session you may choose to exit at any time by using choosing
“Finish Later”, and then return to where you left off when you next
log in. However, PLEASE NOTE that until you press “Survey Complete”
your questionnaire will remain incomplete, and you will
NOT be permitted to participate. DO NOT select the
option “Remove Survey from List” button as it will make you
ineligible to participate.
True Sport Clean 101 E-learning Course For all
varsity athletes (excluding developmental ‘B’ Team and novice
programs).
- All athletes MUST complete the True Sport Clean 101 e-learning
course offered by the Canadian Centre for Ethics in Sport
BEFORE their first practice. For more information
please go to CCES Doping Ed in the left hand menu. See the CCES
Doping Ed page for more information. We are no longer permitted
to publish the access codes on our web site, but they are available
from your coach, your team therapist, from Client Services Office
(Monique G20), from the Intercollegiate Office (Luisa G68) or by
emailing a request to intercollegiate.athletics@mcgill.ca.
- For non-CIS athletes: As this e-learning course was originally
designed for CIS athletes, you will find that your particular non-CIS
sport is not represented. Therefore, please select McGill Other
Sports (Non CIS).
- It is no longer necessary to print or
supply the Intercollegiate Office with a copy of the CCES Certificate
of Completion, but we strongly recommend you save a copy to your hard
drive in case it is ever requested for administrative purposes.
Pre-Participation Questionnaire. Athletes from the
following teams are required to complete a PPPE Questionnaire no
later than August 1st of each year:
- Basketball(m&w)
- Ice hockey(m&w)
- Soccer(m&w)
- Swimming(m&w),
- Football,
- Volleyball(w),
- Lacrosse(m),
- Rugby(m&w),
- Track and field/Cross country(m&w)
- You are obliged to complete the questionnaire in its entirety and
provide responses that are truthful and accurate. Any intentional
misrepresentation of the facts will result in a diminished quality of
care. Please ensure that you submit the information correctly and
indicate which team you are associated with.
- Include your provincial health card or Blue Cross card number,
expiry date, contract and certificate number.
- If you have opted out of the SSMU/PGSS Health plan you are
obligated to provide a list of your current provider, contract and
certificate number.
- If you take any prescription medications provide the prescription
name, dosage and frequency.
- You may choose to complete the questionnaire in a single session
or you may exit at any time by selecting “Finish Later”, and return
to where you left off when you next log in. Please note that until
you press “Survey Complete” your questionnaire will remain
unfinished. DO NOT select the option “Remove Survey from List” button
as it will make you ineligible to participate.
- Your questionnaire must be submitted electronically. Upon
receipt, it will be reviewed by a member of the Sport Medicine Clinic
staff. If deemed necessary, athletes with medical concerns will be
required to complete their physical exam prior to the commencement of
their season.
- Varsity athletes are responsible for being familiar with the two
